What To Have Ready
- NDIS
- NDIS details, relevant medical information, current reports and any information about goals or support needs.
- DVA
- Veteran Gold or White Card, a valid referral, and relevant medical reports and imaging.
- TAC
- TAC claim number, relevant medical reports and imaging, and approvals for care.
- WorkSafe and WorkCover
- Claim number, insurer or agent details, employer information and the current Certificate of Capacity.
